Building and embedding forms
Create intake forms once, then show them as a classic layout or step-by-step conversational screens.
Open Forms in the Desk sidebar to build customer-facing intake forms. The builder is visual: the center canvas looks like the live form, the left side adds fields, and the right side edits settings.
Use Form settings in the editor header anytime — it is always available and is separate from field settings. When you select a field or a step, those settings appear in the side panel, with a clear link back to Form settings.
You define fields once; Desk can present the same form in two ways:
- Standard — all fields on one page. Steps appear as sections.
- Conversational — one step at a time, with progress, Back/Continue, and validation before advancing.
Add Steps to group related fields on the same conversational screen (for example contact details together). A step is different from a heading: headings are visual content inside a step; steps control pagination. Without steps, Conversational still shows one field at a time for older forms.
Field types include text, email, phone, number, choices, dates, file uploads (photos and documents), plus headings and dividers for structure. Map name/email/phone fields for ticket creation when you turn on Create Desk ticket in Form settings.
Publish a form to get a public link at /forms/your-slug on your help center. To place it on your own website, open Share and copy the embed snippet (transparent background, auto-sized height). The embed URL uses ?embed=1 so Help navigation stays hidden.
